My Transition Portfolio

• Steps: First — start a folder (real or digital) with four sections: Who I Am, What I Can Do, What I Have Finished, Where I Am Going. Then — put one real thing in each section (a certificate, a photo of finished work, a reference name, a goal page). Then — write or record your next three steps after this school, in your own order. Creative Studio — Cover Art Project

• Steps: First — pick a piece of your own work (a song, a poem, a story, a beat). Then — design its cover: title, your artist name, one strong image, colors that carry the feeling. Then — make it in any free design app or by hand. Last — write two lines on the back: what the work is, and who it is for.
• You need: paper + colors, or a free design app. No-supplies way: arrange real objects that stand for your work and photograph them as the cover.

My Record, My Proof Folder

• Steps: First — learn the rule from the lesson: keep your own proof, in your own place, where only you control it. Then — build the folder: sections for ID copies, School records, Messages that matter (screenshots), Dates and facts I wrote down. Then — add one real item you already have. Last — write the folder's rule on the front: what goes in, what never does (no passwords, no one else's private information).
• You need: a folder or a private notes album. No-supplies way: one notebook page ruled into the four sections is the whole folder.
• What you have at the end: an organized proof folder — the habit that protects you if you are ever accused.

Know-Your-Rights Wallet Card

• Steps: First — from the lesson, pick the short lines that are yours to say (for example: "I choose to stay silent." "Am I free to go?" "I do not agree to searches."). Then — write or print them on a card that fits a wallet or phone case, with a trusted grown-up's phone number. Then — practice reading or showing the card out loud once, calmly. Last — decide where the card lives, and keep it there.

My Offline Help Pack

• Belongs with: Stand's Offline Packs and Lifelines pages; Never Leave Anyone Stranded (youth library).
• Steps: First — list what you would need if your phone died and you had no signal: key numbers from memory, your address, a meeting place, a help card. Then — write the pack on paper (paper works with no battery). Then — add one page for helping someone else: the numbers and steps from Lifelines. Last — choose where the pack lives (bag, wallet, home door) and tell one trusted person it exists.
• You need: paper + pen. No-supplies way: memorize the three most important numbers and teach them to yourself by saying them in a pattern — the memorized pack counts.
• What you have at the end: a paper help pack that works when nothing else does.
